Partial vs. Full-Thickness Rotator Cuff Tears: What’s the Difference?

If you have been told you have a rotator cuff tear, the next words may have been partial-thickness or full-thickness. They sound similar, but they often lead to very different conversations about treatment.

Start with the anatomy

The rotator cuff is four tendons that wrap over the top of the shoulder and attach muscle to the ball of the arm bone. A partial-thickness tear goes partway through the tendon: some fibers are damaged, but the tendon remains connected to bone. A full-thickness tear goes all the way through, creating a hole in the tendon or pulling it off the bone entirely.

Partial tears are more common than many people realize

Partial-thickness tears are roughly twice as common as full-thickness tears and are a common source of shoulder pain and work disability. Many are found on imaging without causing symptoms, especially in people who perform repeated overhead activity.

Where the partial tear sits matters

Partial tears are commonly described as articular-sided on the underside facing the joint, bursal-sided on the top surface facing the bony arch, or intrasubstance within the tendon. The depth of the tear also matters. The practical dividing line is often 50% of tendon thickness, because it influences treatment discussions.

How symptoms differ — and where they overlap

You cannot reliably tell which kind of tear you have from pain alone. Small partial tears can be very painful, while some large full-thickness tears are surprisingly painless. Pain often reflects inflammation and compensation, not only the amount of tendon damage.

Partial tears more often cause aching pain, overhead discomfort, night pain, and symptoms that develop gradually with preserved strength. Full-thickness tears more often produce true weakness, difficulty lifting the arm, or trouble holding it against resistance. An examination and MRI or ultrasound help clarify the diagnosis.

Do tears get bigger over time?

Some tears progress and some do not. Studies report a wide range of progression for partial tears, with symptoms and activity level playing a role. For full-thickness tears, larger symptomatic tears are more likely to enlarge over time than smaller tears. New or worsening pain in a tear being observed can be a reason to reassess it.

Rotator cuff tendons rarely heal back together on their own. People can improve without surgery when inflammation settles and surrounding muscles compensate, which is a real and legitimate improvement — but it is not the same as the tear closing.

Treatment: where the two really diverge

Partial-thickness tears

Non-surgical care is the starting point in most cases. Physical therapy, activity modification, and selected injection options can improve pain and function. Research supports a genuine trial of non-surgical care even in many tears involving more than 50% of tendon thickness, and some patients improve enough to avoid surgery.

When non-surgical care does not provide adequate relief, debridement may be considered for tears under 50% thickness and repair may be considered for deeper tears. Long-term results after appropriately selected arthroscopic partial tear repair can be durable.

Full-thickness tears

Full-thickness tears require a more individualized discussion. Sudden traumatic tears from a fall, dislocation, or heavy pull deserve prompt surgical evaluation because earlier repair can matter. Gradual degenerative tears usually begin with non-surgical treatment, and many patients improve with physical therapy and a carefully guided plan.

Why waiting is not always free

For larger full-thickness tears, delay can allow the tendon to retract and the muscle to atrophy or develop fatty change. Those changes can be difficult to reverse and may make a repair less reliable or no longer feasible. Earlier surgery deserves careful consideration in younger, active patients with larger repairable tears and limited muscle change.

Initial non-surgical care can often be pursued safely in older patients with chronic tears, in patients with small tears, and in those without a full-thickness tear. The answer to should I wait depends on the tear, age, muscle quality, and how the injury developed.

What this means for your appointment

The partial-versus-full-thickness label is the beginning of the conversation, not the end. Your plan also depends on tear thickness and location, whether the injury was traumatic or degenerative, muscle quality on MRI, age, hand dominance, occupation, strength, range of motion, and your response to therapy so far. Two people with similar MRIs can appropriately choose different paths.

Questions worth asking

  • Is my tear partial or full-thickness, and how deep or large is it?

  • Does my MRI show muscle atrophy or fatty change?

  • Was this caused by an injury or wear-and-tear, and does that change the timeline?

  • If we try therapy first, how long before we reassess?

  • Is there a risk that waiting makes this harder to fix later?

The bottom line

Partial-thickness tears are more common, usually merit a real trial of non-surgical care, and often improve without an operation. Full-thickness tears are more variable: sudden traumatic tears deserve prompt evaluation, while gradual degenerative tears often start with therapy. Larger tears in active patients are where waiting too long can close options.
