Returning to Golf After Shoulder Replacement: Timeline, Recovery Tips, and What to Expect

By Eric J. Sanders, MD – Shoulder and Elbow Surgeon at Texas Orthopaedic Associates

For many patients, the ability to return to an active lifestyle—especially playing golf—is one of the most important goals after shoulder arthroplasty (shoulder replacement surgery). Whether you’ve undergone a total shoulder arthroplasty (TSA) or a reverse total shoulder arthroplasty (rTSA), getting back on the course is absolutely possible—and often with less pain and more enjoyment than before surgery.

But when can you start? How should you progress? And what can you expect from your game?

In this article, I’ll break down my surgeon-recommended golf return protocol, recent research data on return rates and handicaps, and realistic expectations after surgery.

Can You Return to Golf After Shoulder Replacement?

Yes. In fact, most patients are able to return to golf after shoulder arthroplasty. A 2024 study published in JSES International (PMC10894547) found that:

  • 94% of golfers returned to play after anatomic total shoulder arthroplasty (TSA)

  • 76% of golfers returned after reverse total shoulder arthroplasty (rTSA)

  • The majority returned at 4–6 months post-op

  • 94% of those returning had no increase in handicap after surgery

That means for most patients, not only is return to play realistic—it often comes with improved comfort and performance.

Recommended Timeline to Return to Golf After Shoulder Arthroplasty

As a shoulder surgeon and golf enthusiast myself, I generally recommend starting golf-specific activity around 4 months after surgery, with a 6-week progressive return-to-play protocol.

🗓️ Week-by-Week Golf Progression After Shoulder Replacement:

Week Post-Op

Golf Activity

Week 16

Putting only – focus on alignment and distance control

Week 17

Green-side chipping – low-force, short-range swings

Week 18

Wedge shots (50–75 yards) – short swings with higher control

Week 19

Short irons (8–9 iron) – increase swing length and tempo gradually

Week 20

Long irons or hybrids – higher swing effort, full motion

Week 21

Driver and full swings – return to full game play

⚠️ Always consult your surgeon or physical therapist before initiating this progression to ensure appropriate healing of soft tissues and joint structures.

Swing Adjustments and Recovery Tips

After shoulder arthroplasty, a few changes in your swing or equipment may help protect your shoulder and improve performance:

✅ Use Lighter Shafts

Graphite shafts may reduce impact and load on the shoulder.

✅ Shorten Your Backswing

Many patients benefit from a controlled, three-quarter swing—especially in the early stages.

✅ Focus on Smooth Tempo

Avoid quick transitions or explosive torque; a balanced, fluid swing will be more shoulder-friendly.

✅ Consider Professional Swing Evaluation

A session with a golf pro or physical therapist trained in biomechanics can help rework your swing around your new shoulder mechanics.

Differences Between TSA and rTSA in Golf Return

🟢 Total Shoulder Arthroplasty (TSA)

  • Higher return rate (94%)

  • Typically recommended for patients with intact rotator cuffs

  • Allows more normal shoulder mechanics, ideal for consistent swings

🟡 Reverse Total Shoulder Arthroplasty (rTSA)

  • Slightly lower return rate (76%)

  • Designed for patients with rotator cuff tears or severe arthritis

  • May limit external rotation and full swing, but putting and short game remain strong

🎯 Bottom line: Golf is still possible with either surgery type. The approach may be different, but the satisfaction remains.

What About Handicap After Shoulder Replacement?

Good news: most patients return to their pre-surgery golf handicap—or better.

  • In the same 2024 study, 94% of golfers reported no worsening of handicap

  • Many actually reported reduced pain, improved range of motion, and better swing mechanics

With proper rehab and a gradual return, patients often find their swing becomes more repeatable and less painful, leading to greater consistency and enjoyment.

Tips for a Successful Golf Comeback

  • Stick to your rehab plan: Golf uses more shoulder rotation than you think.

  • Respect the 4-month timeline: Too early can increase the risk of implant stress or injury.

  • Warm up before play: Dynamic stretches and short chip shots before hitting the range help prepare the joint.

  • Use your body—not just your arms: Engage hips and core for a shoulder-protective swing.

  • Stay active year-round: Off-season strength and mobility training will keep your game—and shoulder—strong.

Final Thoughts from Dr. Sanders

Shoulder replacement doesn’t mean the end of your golf game—in many cases, it’s the beginning of a better one. Whether you play recreationally or competitively, returning to golf after shoulder arthroplasty is absolutely within reach.

With the right timeline, swing adjustments, and medical guidance, most patients return to the sport they love without pain—and often with a smile.
