


How to Get Better Sleep After Shoulder Surgery (While Still in a Sling)

Recovering from shoulder surgery often means adapting your daily routine—and one of the biggest challenges is getting quality sleep while wearing a sling. Sleep is essential to healing, but pain, stiffness, and limited sleeping positions can make rest difficult during the early stages of recovery.
Fortunately, with a few smart adjustments, you can improve your comfort and rest more easily. Below, we’ll walk through practical ways to sleep better after shoulder surgery while keeping your arm safely immobilized.
1. Why Sleeping Flat Is a Problem
Lying flat after shoulder surgery can increase swelling and tension around the surgical site. It also places strain on your shoulder and makes it harder to keep your arm in a protected, stable position. Most surgeons, including myself, recommend sleeping on an incline for several weeks after surgery to reduce pain and avoid compromising your repair.
2. Elevated Sleeping Options
✅ Foam Wedge in Bed
One of the most accessible solutions is a triangular foam wedge pillow. These are designed to gently elevate your upper body at a 30- to 45-degree angle, helping reduce swelling and keep your shoulder supported. You can place a few small pillows around your sling to prevent rolling in your sleep or to cushion the elbow and wrist.
Tips:
Use additional pillows under your knees to prevent sliding down.
Choose a firm wedge—too soft and you may sink, straining your shoulder.
Consider placing a rolled towel behind your shoulder blade for gentle support.
✅ Adjustable Bed
If you have an adjustable bed at home, this can be ideal. Elevating the head of the bed to a reclining position allows you to sleep in a supported position without needing extra pillows. Most patients find this setup more stable and easier to get in and out of compared to flat beds with multiple pillows.
Tips:
Adjust the leg section slightly to reduce pressure on your lower back.
Make sure your sling is secure, and tuck in a small pillow under your forearm to keep it relaxed.
✅ Recliner Chair
A recliner chair is a favorite option for many shoulder surgery patients, especially during the first 1–2 weeks. It naturally provides an upright position with stable support and can help prevent accidentally rolling onto your surgical side.
Tips:
Choose a recliner with adequate neck and lumbar support.
Drape a thin blanket or towel over your sling-side arm to prevent drafts or irritation.
Keep a side table nearby with essentials like medications, water, and your phone—once you’re comfortable, you’ll want everything within reach.
3. Managing Pain and Swelling Before Bed
Ice Before Sleep: Apply a cold pack (outside the sling and surgical dressing) for 15–20 minutes before bedtime. This can help reduce inflammation and relieve aching.
Medication Timing: Take your prescribed pain medication or anti-inflammatory medication as directed, ideally 30 minutes before sleep. Don’t wait until discomfort wakes you up—prevention is key.
Limit Fluids Late: Try to avoid large amounts of fluid within an hour or two of sleep so you’re not waking up frequently for bathroom trips.
4. Setting a Calming Sleep Environment
Keep the Room Cool: A slightly cool room (around 65–68°F) can make it easier to fall and stay asleep.
Diminish Noise and Light: Use blackout curtains, earplugs, or a white noise machine to block out distractions.
Wear Your Sling: Even while sleeping, it's essential to wear your sling unless your surgeon instructs otherwise. It protects the repair and keeps your arm from moving or falling into harmful positions during the night.
5. What to Expect in the First Few Weeks
Week 1–2: Most patients report sleeping best in a recliner or bed with a foam wedge. Expect to sleep lighter and wake more often—this is normal.
Week 3–4: You may transition to a flatter incline and eventually to your bed with supportive pillows. Continue wearing your sling unless advised otherwise.
Beyond 4 Weeks: Once cleared by your surgeon, you may begin experimenting with side or back sleeping without the sling, using pillow support under the arm.
